We are seeking a dedicated Registered Nurse (RN) with strong home health experience and expertise in OASIS documentation for a 13-week full-time contract assignment. Schedule:
- 13-week assignment with opportunities to extend
- Full-time, Monday through Friday
- Daily visit requirements with potential for on-call responsibilities
Primary Responsibilities:
- Deliver skilled nursing care and interventions to patients in their residences
- Complete thorough patient assessments and timely OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to implement personalized care plans
- Perform wound care, medication administration, patient and family education, and monitor disease progression
- Communicate relevant updates or condition changes to the clinical team promptly
Required Qualifications:
- Active RN license
- Minimum of one year of recent home health RN experience
- Demonstrated OASIS proficiency
- HomeCare HomeBase experience strongly preferred
- Reliable transportation and valid drivers license
- Commitment to a full-time, structured schedule and quality patient care
