What to Expect:

• 13-week contract with potential extension
• Structured visit expectations each day

Key Responsibilities:

• Deliver in-home nursing care and skilled clinical interventions
• Conduct thorough patient assessments, ensuring accurate and timely OASIS documentation
• Collaborate with physicians and the interdisciplinary care team to develop and implement customized care plans
• Perform essential tasks including wound care, medication administration, patient education, and chronic disease monitoring
• Maintain effective communication with the care team regarding changes in patient condition

What Were Looking For:

• Active RN license
• Minimum of one year recent experience as a home health RN
• Proficiency with OASIS documentation is required
• Familiarity with HomeCare HomeBase (strongly preferred)
• Ability to independently manage a full-time, structured caseload
• Reliable transportation and valid drivers license
