This travel nursing opportunity specializes in Home Health, focusing on comprehensive case management in a dynamic, multi-disciplinary environment. Youll play a pivotal role by facilitating communication among patients, families, nurses, physicians, and an extensive care team. Schedule/Hours:

  • Travel position; specific schedule to be confirmed.

Key Responsibilities:

  • Collaborate closely with interdisciplinary teams to develop, revise, and implement patient care plans.
  • Act as a consultative resource for complex clinical situations, focusing on acute and chronic care needs.
  • Coordinate patient care, monitor appropriateness and timeliness of services, and ensure alignment with clinical guidelines and payer requirements.
  • Discuss and evaluate resource utilization, consulting needs, and discharge planning with physicians.
  • Address the needs of high-risk groups such as frequent emergency department users and patients with chronic pain or substance use conditions.
  • Facilitate seamless transfers to tertiary centers, ensuring continuity of information and care.
  • Gather and report regulatory compliance data.
  • Lead studies and reviews of complex or frequent hospital admissions.

Desired Skills & Experience:

  • Active RN license, prior Home Health experience strongly preferred.
  • Strong background in case management, with a collaborative approach to patient advocacy and care coordination.
  • Outstanding communication and analytical skills.
  • Experience managing complex and high-utilization patient groups.
