A nursing professional with expertise in home health care is sought for a contract opportunity in Waipahu, HI. This role involves providing exceptional patient care through effective collaboration and management of a comprehensive case management program. The nurse will act as a key liaison among patients, families, physicians, nurses, and the multidisciplinary healthcare team to optimize patient outcomes and ensure seamless care coordination.

Key Responsibilities:

• Lead coordination of patient care across disciplines, ensuring timely and appropriate interventions.
• Monitor and update interdisciplinary care plans to align with patients clinical conditions and ongoing care needs.
• Collaborate with physicians on resource utilization, treatment plans, length of stay assessments, and discharge planning.
• Manage complex cases, including frequent emergency department utilizers, chronic pain and substance abuse patients, and homeless populations.
• Work closely with acute care case managers to ensure continuity and appropriateness of ongoing care.
• Coordinate patient transfers to tertiary care centers, ensuring complete and accurate transfer of medical information.
• Oversee data collection related to regulatory and accreditation compliance.
• Conduct review studies for patients with frequent emergency department visits to improve care outcomes.
