Take on an impactful nursing role focused on delivering direct patient care in a home health setting. This position is offered on a full-time, 13-week contract basis, ideal for Registered Nurses who excel in independent environments and thrive on meaningful patient relationships.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Role Highlights:

• Manage a caseload aligned to 30 points per week (typically 35 patient visits per day)
• Field-based care, serving patients in their homes within the assigned territory
• Mileage reimbursement provided for daily travel

Key Responsibilities:

• Conduct comprehensive OASIS assessments and create individualized care plans for each patient
• Deliver skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease support
• Closely monitor patient progress, promptly communicating status changes to the interdisciplinary care team and physicians
• Educate patients and families about diagnoses, treatments, medication regimens, and preventive strategies
• Document all patient care accurately and promptly using the Home Care Home Base (HCHB) system
• Collaborate effectively with physical therapists, occupational therapists, social workers, and home health aides to coordinate holistic care
• Participate in team meetings, ongoing education, and case conferences
• Maintain compliance with all regulatory and agency standards

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent field-based Home Health experience
• Hands-on proficiency with Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, a valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
