Your next nursing opportunity starts here. A full-time Registered Nurse is needed for a 13-week home health contract, caring for patients within their homes and making a meaningful difference every day. Clinical excellence, independent time management, and accurate documentation are essential for success in this role.
Schedule:
- Full-time, Monday through Friday
- On-call participation may be required
Key Responsibilities:
- Deliver in-home patient care, including assessments, skilled interventions, and wound care
- Complete OASIS documentation promptly and accurately
- Collaborate effectively with physicians and multidisciplinary teams to create individualized care plans
- Provide patient education on health management and disease prevention
- Oversee medication administration and closely monitor patient conditions
- Communicate promptly regarding changes in patient status and update the clinical team as needed
Qualifications:
- Current, active RN license
- At least 1 year of recent experience as a home health RN
- Experience completing OASIS documentation is required
- Familiarity with HomeCare HomeBase (highly preferred)
- Demonstrated ability to organize and manage a demanding, structured schedule
- Reliable transportation and a valid drivers license
