Take your nursing expertise on the road and provide essential care where it matters most. We're seeking a Registered Nurse with proven OASIS experience for a 13-week home health contract focusing on patient-centered care and clinical documentation excellence.
Schedule:
- Full-time, Monday through Friday
- On-call rotation as needed
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care, including wound treatment, medication administration, patient education, and disease management
- Accurately perform patient assessments and complete OASIS documentation within required timeframes
- Partner with physicians and interdisciplinary care teams to implement individualized care plans
- Maintain thorough communication on patient condition and care updates to clinical management
- Navigate a dynamic caseload while upholding high standards of care and compliance
Qualifications:
- Current and active RN license
- Minimum 1 year of recent home health RN experience (required)
- Demonstrated proficiency with OASIS documentation
- HomeCare HomeBase EMR experience (strongly preferred)
- Consistent ability to manage a full-time schedule and structured patient visits
- Valid drivers license and dependable transportation
