Your next nursing opportunity starts here. We are seeking a committed Registered Nurse with strong OASIS experience for a 13-week full-time home health contract. This position offers the chance to make a meaningful impact, working within patients homes while managing your caseload with precision and care.
Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday
- On-call responsibilities may be included
- Structured daily visit requirements
Key Responsibilities:
- Deliver skilled in-home nursing care, including wound care, medication management, and disease monitoring
- Conduct comprehensive patient assessments and complete OASIS documentation accurately and timely
- Implement and revise individualized care plans in coordination with physicians and care teams
- Provide patient and caregiver education to support optimal outcomes
- Maintain clear communication regarding patient status and changes to the clinical team
Qualifications:
- Active RN license
- Minimum of 1 year recent home health RN experience
- OASIS proficiency required
- Experience with HomeCare HomeBase strongly preferred
- Reliable transportation and a valid drivers license
- Demonstrated ability to follow a structured, full-time visit schedule
