Step into a pivotal nursing role focused on providing high-quality care for patients in their homes. This full-time, 13-week contract assignment specializes in home health services, empowering registered nurses to deliver hands-on care and education while working independently in the field.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN license in good standing for the state of practice
• At least 1 year of recent home health nursing experience; field experience strongly preferred
• Proven proficiency with Home Care Home Base (HCHB) electronic documentation
• Solid experience with OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

Primary Responsibilities:

• Complete thorough OASIS assessments and devise patient-centered care plans tailored to individual needs
• Deliver skilled nursing tasks such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient status and promptly communicate any changes to the care team or physicians
• Educate patients and families on health conditions, treatments, medications, and preventive measures
• Record documentation accurately and efficiently using HCHB
• Collaborate closely with interdisciplinary team members, including therapists, social workers, and aides, to ensure seamless care
• Participate in case conferences, staff meetings, and ongoing professional education
• Uphold regulatory and agency standards for high-quality patient care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
