Embark on a rewarding full-time opportunity providing compassionate care to patients in their homes while advancing your professional nursing skills. This 13-week contract offers a structured weekday schedule and the ability to make a meaningful impact as a field-based RN dedicated to home health.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop tailored patient care plans.
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor and evaluate patient outcomes, promptly report condition changes, and ensure seamless communication with the broader care team and physicians.
• Educate patients and families on disease management, medications, and preventative strategies to promote health and independence.
• Accurately document all care provided using Home Care Home Base (HCHB).
• Collaborate closely with physical therapists, occupational therapists, medical social workers, and home health aides for fully coordinated care.
• Participate in case conferences, team meetings, and continuous education opportunities.
• Uphold rigorous compliance with regulatory and agency documentation and care delivery standards.

Requirements:

• Active RN license (state-specific and in good standing)
• Minimum of 1 year of recent home health nursing experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB)
• OASIS experience required
• Reliable transportation, valid drivers license, and proof of auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
