Deliver high-quality patient care as a Registered Nurse within a field-based home health setting, providing vital support to individuals in their own residences for a 13-week full-time contract. This opportunity offers the chance to build meaningful relationships and make a genuine impact during every patient visit.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Daily travel to patient homes within your assigned territory (mileage reimbursement provided)
• Expect to manage a caseload of 30 points per week, typically conducting 35 patient visits each day

Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing services, including medication administration, wound care, IV therapy, and management of chronic diseases
• Track patient progress closely, communicate changes to physicians and the multidisciplinary team, and promptly update care plans as needed
• Educate patients and their families on diagnoses, medication regimens, treatment plans, and preventive care strategies
• Accurately document all care and communications using Home Care Home Base (HCHB) software
• Collaborate with therapists, medical social workers, and home health aides to optimize outcomes
• Participate in team meetings, case conferences, and required continuing education
• Uphold all agency and regulatory standards for home health care documentation and service delivery

Qualifications:

• Current, active RN license in good standing (state of practice)
• Minimum 1 year recent home health nursing experience (field-based strongly preferred)
• Required proficiency in Home Care Home Base (HCHB) and use of OASIS assessment tools
• Valid drivers license, reliable transportation, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
