We are seeking an experienced Registered Nurse with a passion for home health care and expert-level OASIS documentation for a full-time, 13-week contract assignment. Schedule:
- Full-time, Monday through Friday
- On-call rotation may be required

Key Responsibilities:
- Deliver skilled nursing care in patient homes, ensuring compassionate and individualized treatment
- Perform patient assessments, develop care plans, and complete OASIS documentation accurately and promptly
- Liaise with physicians and multidisciplinary care teams to coordinate and optimize patient care
- Provide wound care, medication administration, disease monitoring, and patient/family education
- Communicate timely updates and any changes in patient condition to the clinical team
Who You Are:
- Active and unencumbered RN license
- Minimum of 1 year recent home health RN experience
- Demonstrated OASIS proficiency required
- HomeCare HomeBase experience strongly preferred
- Able to consistently manage a structured, full-time patient visit schedule
- Reliable transportation and valid drivers license
