Job Description

An opportunity is available for a skilled Registered Nurse (RN) specializing in home health to provide compassionate, patient-centered care on a contract basis. This role focuses on delivering nursing services in patients homes, emphasizing individualized care and collaboration with healthcare teams.

Key responsibilities include:

  • Conducting comprehensive assessments, including vital signs, medication management, and wound care.
  • Developing and implementing tailored care plans responsive to patient needs.
  • Educating patients and families on disease management, prevention, and treatment adherence.
  • Collaborating closely with physicians, therapists, and other professionals to ensure cohesive care.
  • Maintaining precise and timely documentation within electronic medical records (EMR).
  • Monitoring patient conditions and communicating any changes to supervising physicians.
  • Adhering to regulatory standards and compliance requirements pertinent to home health.

Qualifications and desired experience:

  • Active Registered Nurse (RN) license.
  • Minimum of three months home health experience utilizing OASIS assessments.
  • Experience with Homecare Home Base (HCHB) charting system is highly desirable.
  • Proficiency in clinical skills such as IV therapy, wound care, and medication administration.
  • Skilled in electronic medical records (EMR) usage.
  • Strong communication skills and ability to work independently.
  • Valid drivers license and reliable transportation.
