An opportunity is available for a skilled Registered Nurse (RN) specializing in home health to provide compassionate, patient-centered care on a contract basis. This role focuses on delivering nursing services in patients homes, emphasizing individualized care and collaboration with healthcare teams.
Key responsibilities include:
- Conducting comprehensive assessments, including vital signs, medication management, and wound care.
- Developing and implementing tailored care plans responsive to patient needs.
- Educating patients and families on disease management, prevention, and treatment adherence.
- Collaborating closely with physicians, therapists, and other professionals to ensure cohesive care.
- Maintaining precise and timely documentation within electronic medical records (EMR).
- Monitoring patient conditions and communicating any changes to supervising physicians.
- Adhering to regulatory standards and compliance requirements pertinent to home health.
Qualifications and desired experience:
- Active Registered Nurse (RN) license.
- Minimum of three months home health experience utilizing OASIS assessments.
- Experience with Homecare Home Base (HCHB) charting system is highly desirable.
- Proficiency in clinical skills such as IV therapy, wound care, and medication administration.
- Skilled in electronic medical records (EMR) usage.
- Strong communication skills and ability to work independently.
- Valid drivers license and reliable transportation.
