Schedule & Hours:
- Full-time, Monday through Friday
- On-call likely required
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions to patients
- Conduct comprehensive assessments and accurately complete timely OASIS documentation
- Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans
- Perform wound care, medication administration, patient education, and disease monitoring
- Clearly communicate patient status updates and significant changes to the broader clinical team
Required Qualifications:
- Active Registered Nurse license
- At least one year of recent experience as a home health RN
- Previous OASIS documentation experience
- Proficiency with HomeCare HomeBase (strongly preferred)
- Ability to manage a consistent, full-time field schedule with daily patient visits
- Reliable transportation and a valid drivers license
