Schedule:
- Full-time, Monday through Friday
- On-call duties likely
- Structured daily visit requirements
Qualifications:
- Active RN license required
- At least one year of recent experience as a home health RN
- Proficiency with OASIS documentation is mandatory
- Previous use of HomeCare HomeBase strongly preferred
- Strong clinical skills, particularly in wound care, medication management, and patient education
- Reliable transportation and valid drivers license
Responsibilities:
- Deliver in-home nursing care based on individualized care plans
- Complete thorough patient assessments and all OASIS documentation accurately and on schedule
- Collaborate closely with physicians and care teams to achieve the best outcomes
- Administer skilled interventions such as wound care, medication management, and disease monitoring
- Maintain clear and timely communication on patients progress or changes in condition
