Schedule & Expectations:
- 13-week contract (potential for extension)
- Full-time, Monday through Friday
- Daily visit expectations
- On-call rotation likely
Responsibilities:
- Deliver in-home skilled nursing services with compassion and accuracy
- Conduct patient assessments and complete timely OASIS documentation
- Administer wound care, monitor medications, and educate patients/families
- Report changes in patient conditions to the broader care team
- Collaborate with physicians and allied healthcare professionals to tailor care plans
- Maintain accurate visit records and comply with all regulations
Qualifications:
- Active RN license
- At least one year of recent home health RN experience
- Demonstrated proficiency with OASIS documentation
- Experience using HomeCare HomeBase is strongly preferred
- Ability to handle a full-time structured caseload and daily travel
- Reliable transportation with a valid drivers license
