We're seeking a dedicated Registered Nurse (RN) with proven OASIS experience for a 13-week full-time assignment in home health. Schedule/Hours:

• 13-week contract with the possibility for extension
• Full-time, Monday through Friday
• On-call responsibilities may be included
• Structured daily visit expectations

Key Responsibilities:

• Deliver skilled nursing care to patients in their homes, following individualized care plans
• Complete patient assessments and timely, accurate OASIS documentation
• Collaborate with physicians and interdisciplinary care teams to optimize patient outcomes
• Provide wound care, medication management, patient education, and monitor disease progression
• Communicate patient condition updates and changes promptly to the clinical team

Qualifications:

• Current, active RN license
• At least one year of recent home health RN experience
• Demonstrable proficiency with OASIS documentation
• Familiarity with HomeCare HomeBase preferred
• Comfortable managing a consistent full-time schedule with structured patient visits
• Possess reliable transportation and valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
