We are seeking a Registered Nurse (RN) with OASIS expertise for a 13-week full-time contract focused on delivering outstanding patient care in the home setting. Schedule:
- Full-time, Monday through Friday
- On-call responsibilities are likely
- Daily visit expectations with mileage reimbursement between visits
Key Responsibilities:
- Deliver skilled in-home nursing care, including wound care, medication management, disease monitoring, and patient education
- Conduct thorough patient assessments and consistently complete OASIS documentation with accuracy and timeliness
- Collaborate with physicians and an interdisciplinary care team to develop and implement individualized care plans
- Effectively communicate updates or changes in patient conditions to the broader clinical team
- Adhere to compliance and safety standards while managing daily visits efficiently
Qualifications:
- Active RN license in good standing
- At least 1 year of recent experience as a Home Health RN
- Proven proficiency with OASIS assessments and documentation
- Experience with HomeCare HomeBase is strongly preferred
- Reliable vehicle, valid drivers license, and ability to manage a structured schedule
