We are seeking a skilled and compassionate Registered Nurse (RN) with solid home health and OASIS experience to join a dedicated care team for a 13-week full-time contract assignment. Schedule:
- 13-week contract, with the potential for extension
- Full-time, Monday through Friday
- On-call may be required
- Structured daily home health visits
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions
- Complete patient assessments and OASIS documentation accurately and promptly
- Collaborate effectively with physicians and interdisciplinary team members to create and implement individualized care plans
- Provide wound care, manage medications, offer patient and caregiver education, and monitor chronic illnesses
- Maintain clear, timely communication with the clinical team regarding patient status and changes
Required Qualifications:
- Current active RN license
- At least one year of recent experience as a home health RN
- Demonstrated proficiency with OASIS documentation
- HomeCare HomeBase software experience is strongly preferred
- Reliable transportation and a valid drivers license
- Strong organizational skills and ability to self-manage a full patient caseload
