Embark on a rewarding opportunity delivering skilled nursing care in a field-based home health environment. This full-time, 13-week contract role offers the chance to make a significant impact on patient outcomes while working independently and collaboratively as part of a multidisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Role Overview:

• Manage a caseload of approximately 30 points per week, typically resulting in 35 patient visits daily
• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor and evaluate patient progress, promptly addressing changes in condition and communicating with the care team and physicians
• Educate patients and families on disease management, medications, and preventive health measures
• Complete accurate and timely documentation using Home Care Home Base (HCHB)
• Collaborate effectively with physical therapists, occupational therapists, medical social workers, and home health aides to ensure seamless, coordinated care
• Participate in case conferences, staff meetings, and continuing education sessions
• Ensure strict adherence to regulatory guidelines and agency standards

What Youll Need:

• Active RN license in good standing for the state of practice
• At least 1 year of recent home health experience, with a preference for field work
• Proficiency with the Home Care Home Base (HCHB) system
• Demonstrated experience with OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
