Advance your nursing career in a field-based role serving patients in their homes. This full-time, 13-week contract involves providing high-quality care, developing comprehensive care plans, and working collaboratively with multidisciplinary teams.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based role with daily travel to patient homes in the assigned territory
• Caseload expectation: 30 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Perform thorough OASIS assessments and develop individualized care plans tailored to each patient
• Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress and promptly communicate any changes to physicians and the broader care team
• Educate patients and families about disease processes, medications, treatment plans, and preventive care
• Document all patient care and visits accurately and efficiently using Home Care Home Base (HCHB)
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for coordinated care delivery
• Attend case conferences, staff meetings, and participate in ongoing education as needed
• Comply with all regulatory and agency standards regarding documentation and patient care

Qualifications:

• Active RN license in the state of practice, in good standing
• At least one year of recent home health experience (field experience preferred)
• Proficiency in OASIS and Home Care Home Base (HCHB) required
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
