A dedicated and experienced Registered Nurse is needed for a full-time, 13-week home health contract. This role calls for exceptional clinical skills, meticulous documentation, and the ability to manage a set scheduleoffering a meaningful way to make a difference in patients lives through home-based care.
Schedule/Hours:
- 13-week contract duration, with potential to extend
- Full-time, Monday through Friday
- Possible on-call responsibilities
- Daily visit metrics to be met
Key Responsibilities:
- Deliver high-quality, in-home nursing care and skilled interventions
- Complete comprehensive patient assessments with timely and accurate OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to create and implement individualized plans of care
- Provide wound care, manage medications, educate patients, and monitor disease progression
- Clearly communicate patient needs and condition changes to the clinical team
Preferred Qualifications:
- Current and active RN license
- At least 1 year of recent home health RN experience required
- OASIS documentation experience is essential
- Strong proficiency with HomeCare HomeBase is highly preferred
- Demonstrated ability to manage a full-time, structured schedule with daily patient visits
- Reliable transportation and valid drivers license required
