We are hiring a dedicated Registered Nurse with OASIS experience for a full-time, 13-week home health assignment. Schedule:
- 13-week contract, potential for extension
- Full-time Monday through Friday
- On-call responsibilities may apply
- Structured daily patient visits
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions
- Perform comprehensive patient assessments and complete accurate, timely OASIS documentation
- Work closely with physicians and interdisciplinary teams to implement individualized care plans
- Execute wound care, medication administration, patient education, and chronic disease monitoring
- Maintain clear communication regarding patient status with the clinical team
Qualifications:
- Current and active RN license
- At least one year of recent home health RN experience
- Demonstrated OASIS documentation expertise (required)
- Strong proficiency with HomeCare HomeBase (preferred)
- Ability to manage a structured full-time schedule and handle daily patient visits
- Reliable vehicle and valid drivers license
