Schedule:
- 13-week contract, with potential to extend
- Full-time, Monday through Friday schedule
- Probable on-call responsibilities as needed
- Structured daily visit expectations
Core Responsibilities:
- Deliver skilled nursing care directly to patients in their homes
- Complete comprehensive patient assessments, ensuring accurate and timely OASIS documentation
- Coordinate with physicians and interdisciplinary team members to support individualized care
- Provide wound care, medication management, disease monitoring, and patient education
- Update the clinical team promptly on patient condition changes
Desired Skills and Experience:
- Active, unrestricted RN license
- Minimum of one year recent experience as a home health RN
- OASIS documentation experience required
- Proficiency with HomeCare HomeBase (strongly preferred)
- Ability to consistently manage a structured, full-time schedule with daily patient visits
- Reliable transportation and a valid drivers license
