An exciting 13-week full-time contract is available for a skilled Registered Nurse with expertise in home health and OASIS documentation. This position is ideal for a nurse who thrives in a dynamic environment, enjoys patient-centered care, and delivers accurate documentation with strong time management. Your clinical aptitude will directly enhance patient outcomes and support an empowered, purposeful care team.
Schedule:
- Monday through Friday, full-time hours
- On-call duties may be required
Key Responsibilities:
- Deliver comprehensive in-home nursing care and skilled clinical interventions
- Conduct patient assessments and accurately complete timely OASIS documentation
- Collaborate with physicians and multidisciplinary care teams to develop and execute individualized patient care plans
- Provide wound care, medication management, disease monitoring, and patient/family education
- Communicate promptly with the clinical team regarding status changes and care updates
- Meet structured daily visit expectations efficiently and compassionately
Preferred Qualifications:
- Active, unencumbered RN license
- At least 1 year of recent home health RN experience
- Demonstrated proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase software (strongly preferred)
- Consistent reliability, organizational strength, and ability to manage scheduled patient visits
- Valid drivers license and reliable transportation
