A skilled Registered Nurse with a strong background in home health is needed for a 13-week full-time contract assignment. If you excel in OASIS documentation, clinical decision-making, and managing a dynamic weekday schedule, this position offers the chance to make a real difference in patients lives.

Schedule:

  • 13-week contract with the possibility of extension
  • Full-time hours, Monday through Friday
  • On-call participation may be required
  • Structured patient visit volume each day

Key Responsibilities:

  • Deliver in-home nursing care, including patient assessments and skilled interventions
  • Accurately complete OASIS documentation in a timely manner
  • Partner with physicians and interdisciplinary care teams to design and deliver individualized patient care plans
  • Perform wound care, medication management, disease surveillance, and patient education
  • Clearly communicate all updates and clinical changes to the wider care team to ensure the highest standard of care

Desired Qualifications:

  • Current, active RN license
  • At least one year of recent home health nursing experience
  • Proven OASIS documentation proficiency
  • Experience with HomeCare HomeBase (strongly preferred)
  • Valid drivers license and reliable vehicle for daily travel
  • Ability to manage a steady, structured caseload with consistent patient visits
