Job Description
Seeking an experienced RN with a passion for delivering high-quality care in a home health setting. This full-time, 13-week contract offers the opportunity to make a direct impact on patients lives through skilled nursing services provided in their homes.
Schedule & Work Environment
- Monday Friday, 8:00 AM 5:00 PM
- Field-based visits to patient homes within an assigned territory
- Travel required; mileage reimbursement is offered
- Caseload: 30 points per week, typically 35 patient visits per day
What Youll Do
- Complete comprehensive OASIS assessments and develop individualized care plans for each patient
- Administer skilled nursing interventions, such as wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, swiftly identifying and reporting changes in condition to the care team and physicians
- Deliver patient and family education regarding disease process, medications, treatment plans, and preventive care
- Maintain meticulous, timely documentation using Home Care Home Base (HCHB)
- Collaborate effectively with a multidisciplinary teamincluding PT/OT, medical social workers, and home health aidesto ensure seamless care coordination
- Attend case conferences, participate in continuing education, and adhere to all regulatory and agency standards for documentation and care delivery
What Were Looking For
- Current, active RN license in the state of practice (good standing)
- At least 1 year of recent field-based Home Health nursing experience
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Reliable transportation, a valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
