A full-time, 13-week contract nursing role is available for an experienced RN skilled in Home Health care. This field-based position focuses on delivering skilled nursing services to patients in their homes, utilizing OASIS assessments and electronic documentation via Home Care Home Base (HCHB). The contract requires reliable daily travel within your assigned territory and provides mileage reimbursement.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Role Highlights:

• Manage a caseload targeted at 30 points per week (generally 35 patient visits daily)
• Conduct initial and ongoing OASIS assessments, developing individualized plans of care
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and support with chronic conditions
• Closely monitor patient progress, recognize changes, and communicate with physicians and the interdisciplinary team
• Educate patients and their families on disease management, medications, and preventive practices
• Ensure thorough, timely documentation in Home Care Home Base (HCHB)
• Engage collaboratively with therapy, social work, and aide staff as part of a coordinated care team
• Attend case conferences, staff meetings, and pursue professional development as required
• Uphold all documentation, regulatory, and agency care standards

Qualifications:

• Current RN license (in good standing)
• At least 1 year of recent Home Health field experience
• Demonstrable proficiency with Home Care Home Base (HCHB)
• Direct experience with OASIS assessments
• Reliable transportation, valid drivers license, and automobile insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
