A full-time contract is available for a skilled Registered Nurse with proven home health and OASIS experience. This 13-week assignment offers structure, a supportive team, and the chance to make a meaningful difference for patients in the comfort of their homes.

Schedule:

• Monday through Friday, full-time hours
• On-call responsibilities may be required

Key Responsibilities:

• Deliver compassionate in-home nursing care and skilled therapeutic interventions
• Conduct thorough patient assessments and complete OASIS documentation with accuracy and timeliness
• Collaborate closely with physicians and interdisciplinary care teams to customize patient care plans
• Perform wound care, medication administration, patient and caregiver education, and ongoing disease monitoring
• Relay patient condition changes to the appropriate clinical staff to ensure high standards of care

Desired Qualifications:

• Current, active RN license
• At least 1 year of recent experience as a home health RN
• Strong OASIS assessment and documentation skills (required)
• Experience with HomeCare HomeBase software (strongly preferred)
• Reliable transportation and a valid drivers license
• Ability to manage and adhere to a set schedule with daily patient visits
