Schedule Details:

What Youll Do:

• Deliver skilled nursing care in home settings, ensuring all interventions meet clinical standards
• Perform thorough patient assessments and complete OASIS documentation with precision and timeliness
• Collaborate closely with physicians and interdisciplinary team members to develop and implement patient-specific care plans
• Administer wound care, manage medications, educate patients and families, and monitor for changes in health status
• Document and communicate patient updates and care needs to the broader clinical team

Required Qualifications:

• Active Registered Nurse license
• At least 1 year of recent experience in home health as an RN
• Strong proficiency in OASIS documentation
• Experience with HomeCare HomeBase (highly preferred)
• Demonstrated ability to work independently on a structured daily schedule of patient visits
• Reliable transportation and a valid drivers license
