An exciting opportunity is available for an experienced Registered Nurse specializing in Home Health. This position is a full-time, 13-week contract, focusing on providing field-based care to patients in their homes throughout your assigned territory.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services, including wound care, IV therapy, chronic disease management, and medication administration.
• Monitor patient progress, recognize any changes in condition, and communicate effectively with physicians and the wider care team.
• Educate patients and their families on disease processes, medications, treatments, and preventive care.
• Accurately document all care and assessments within the Home Care Home Base (HCHB) system.
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides to provide cohesive, coordinated care.
• Take part in case conferences, team meetings, and required continuing education activities.
• Adhere strictly to regulatory and agency documentation and care standards.
• Manage a caseload averaging 30 points per week (typically 35 patient visits daily).
• Participate in daily travel to patient residences within the assigned area (mileage reimbursement provided).

Qualifications:

• Active RN License (in good standing) for the state of practice.
• At least one year of recent Home Health experience (field work preferred).
• Proven ability to use Home Care Home Base (HCHB) for documentation.
• OASIS proficiency required.
• Reliable transportation, a valid drivers license, and current auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
