Deliver skilled nursing care in a home health setting, supporting patients during their recovery and rehabilitation journey. Qualifications and Experience:

• Active RN license in good standing
• Previous experience in home health or community-based nursing preferred
• Strong assessment, clinical, and documentation skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage a varied caseload
• CPR certification

Responsibilities:

• Perform patient assessments and develop individualized care plans
• Administer medications and treatments as prescribed
• Educate patients and families on care, medication management, and health maintenance
• Collaborate with interdisciplinary team members to coordinate patient services
• Monitor patient progress and update care plans as needed
• Maintain accurate, timely clinical documentation in compliance with agency and regulatory standards
• Ensure patient safety and advocate for patient needs
