A professional, mission-driven healthcare team is seeking a dedicated Registered Nurse (RN) with proven home health experience for a 13-week full-time contract in the Sun Valley, NV area. Schedule:
- 13-week contract with the possibility of extension
- Full-time, Monday through Friday
- On-call responsibilities are probable as part of the schedule
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled medical interventions to patients.
- Complete thorough assessments and OASIS documentation with precision and timeliness.
- Collaborate closely with physicians and interdisciplinary care teams to design individualized care plans.
- Perform wound care, medication management, patient and caregiver education, and disease monitoring in the home environment.
- Clearly communicate updates and significant changes in patient status to the larger clinical team.
Qualifications:
- Current and unrestricted RN license is required.
- At least one year of recent professional experience as a home health RN.
- Demonstrated proficiency with OASIS documentation is essential.
- Experience using HomeCare HomeBase is strongly preferred.
- Capable of maintaining a reliable, structured schedule and managing daily visit requirements.
- Must have reliable transportation and a valid drivers license.
