A full-time, 13-week contract assignment is available for an experienced Registered Nurse (RN) specializing in home health care. This role allows you to make a meaningful difference in patients lives through skilled nursing care delivered directly to their homes. Schedule:
- 13-week contract, with option to extend
- Full-time, Monday through Friday
- Daily visit structure

Key Responsibilities:
- Deliver high-quality, in-home nursing care and skilled interventions
- Conduct thorough patient assessments and complete OASIS documentation accurately and promptly
- Collaborate with physicians and care team members to create individualized patient care plans
- Provide wound care, perform medication management, educate patients, and monitor disease progression
- Communicate promptly regarding patient condition changes and coordinate care with the clinical team
Qualifications:
- Current and active RN license
- Minimum of 1 year of recent home health experience as an RN
- Proficient with OASIS documentation (required)
- Experience with HomeCare HomeBase is strongly preferred
- Able to manage a structured daily schedule and meet visit goals
- Reliable transportation and a valid drivers license
