Schedule:

Role Overview:
- Deliver in-home patient care and skilled interventions tailored to individual needs
- Complete timely and accurate OASIS documentation for effective care management
- Coordinate with physicians and interdisciplinary care teams to establish personalized care plans
- Provide wound care, medication administration, patient education, and monitoring for chronic conditions
- Maintain open communication regarding status changes with the clinical team
Preferred Background:
- Unrestricted, active RN license
- At least one year of recent experience in a home health RN position
- Demonstrated OASIS assessment and documentation competency
- Proficiency with HomeCare HomeBase is highly preferred
- Strong organizational skills to handle a dedicated case load and daily visits
- Reliable transportation and a valid drivers license are essential
