We are seeking a skilled Registered Nurse with proven home health experience and a solid background in OASIS documentation for a 13-week full-time contract. This assignment is ideal for a clinician who excels with independent patient care, accurate documentation, and collaboration across interdisciplinary teams.
Schedule:
- Full-time, Monday through Friday
- On-call duties may be required
- Daily structured patient visits
Key Responsibilities:
- Deliver comprehensive in-home nursing care and skilled interventions to patients
- Complete thorough patient assessments and submit timely, accurate OASIS documentation
- Collaborate with physicians and interdisciplinary teams to tailor individualized care plans
- Perform wound care, medication management, disease observation, and patient/family education
- Monitor and communicate any changes in patient condition to the clinical team
Qualifications:
- Active Registered Nurse (RN) license
- Minimum one year of recent home health RN experience
- Extensive experience with OASIS documentation
- Familiarity with HomeCare HomeBase software (highly preferred)
- Consistent and reliable in managing a full-time, structured visit schedule
- Possession of a valid drivers license and dependable transportation
