Schedule:
- Full-time: Monday through Friday
- On-call responsibilities may be included
Role Overview:
- Provide comprehensive in-home nursing care, including skilled interventions
- Accurately conduct patient assessments and complete OASIS documentation within required timelines
- Partner with physicians and interdisciplinary teams to tailor and implement individualized care plans
- Deliver wound care, medication administration, disease monitoring, and thorough patient/family education
- Report patient updates or status changes promptly to the clinical team
- Ensure quality standards by managing daily patient visits efficiently
Qualifications:
- Active and unrestricted RN license
- Minimum 1 year of recent experience as a home health RN
- Established OASIS documentation proficiency (required)
- Familiarity with HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Ability to adhere to a structured schedule and navigate daily visits independently
