A rewarding contract position is available for a skilled Registered Nurse with OASIS experience, ready to deliver exceptional care within the home health setting. This 13-week, full-time assignment offers the chance to make a real difference in patients lives while enjoying the structure of a MondayFriday schedule.
Schedule:
- 13-week contract, with extension potential
- Full-time, Monday through Friday
- Anticipate possible on-call requirements
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions
- Complete comprehensive patient assessments and OASIS documentation with accuracy and timeliness
- Collaborate closely with physicians and multidisciplinary care teams to implement personalized care plans
- Provide wound care, medication management, chronic disease monitoring, and patient education
- Clearly communicate updates and any changes in patient conditions to the clinical team
Qualifications:
- Active RN license (required)
- Minimum one year of recent home health RN experience
- OASIS documentation experience
- Strong proficiency with HomeCare HomeBase (preferred)
- Reliable transportation and a valid drivers license
- Ability to manage a structured schedule involving daily patient visits
