Qualifications:

• Active and unrestricted RN license
• Experience in home health or community-based nursing strongly preferred
• Strong assessment, care planning, and documentation skills
• Comfortable working independently and managing a field-based caseload
• Basic life support (BLS) certification
• Exceptional communication and interpersonal abilities
• Reliable transportation and current drivers license

Key Responsibilities:

• Conduct patient assessments and develop individualized care plans
• Administer prescribed nursing care and monitor patient progress
• Educate patients and families about condition management, medication administration, and preventive health
• Document patient care activities per regulatory and organizational standards
• Collaborate with multidisciplinary teams to coordinate patient care and transitions
