Job Description

Deliver vital patient care in a field-based setting, supporting individuals and families in their homes over a 13-week contract. This full-time role focuses on direct patient interaction, clinical leadership, and interdisciplinary collaboration for ongoing, impactful home health care.

Schedule/Hours:

  • Monday Friday, 8:00 AM 5:00 PM

Role Highlights:

  • Manage a caseload of approximately 30 points per week (typically 35 patient visits daily)
  • Conduct comprehensive OASIS assessments and develop individualized care plans
  • Perform skilled nursing duties such as wound care, medication administration, IV therapy, and chronic disease management
  • Monitor and evaluate patient progress, identifying and reporting any changes in condition promptly
  • Educate patients and their families on disease processes, medication, treatment plans, and preventive care strategies
  • Document all patient information and clinical interactions using Home Care Home Base (HCHB)
  • Coordinate care as part of an interdisciplinary team, working closely with therapists, social workers, and home health aides
  • Participate actively in case conferences, staff meetings, and ongoing training and education
  • Ensure compliance with all clinical documentation, regulatory, and agency standards
  • Travel daily to patient homes within the assigned territory (mileage reimbursement provided)

Qualifications:

  • Current and active RN license in good standing for the state of practice
  • At least 1 year of recent home health nursing experience (field work preferred)
  • Proficiency with Home Care Home Base (HCHB) and experience with OASIS documentation
  • Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
