This opportunity offers rewarding work as a field-based registered nurse specializing in home health over a 13-week, full-time contract. As an essential member of the care team, you will deliver direct patient care in home settings using your clinical expertise and documentation skills.

Schedule:

• Full-time, Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized patient care plans.
• Deliver skilled nursing care (including wound care, medication administration, IV therapy, and chronic disease management) in patients homes.
• Monitor patient status, identify and respond to changes in condition, and communicate promptly with physicians and clinical teams.
• Educate patients and families about treatment plans, medications, and preventive health measures.
• Maintain up-to-date, accurate documentation using Home Care Home Base (HCHB) electronic system.
• Collaborate actively with interdisciplinary team members (physical therapy, occupational therapy, medical social work, and home health aides) to coordinate care.
• Engage in case conferences, staff meetings, and participate in continuing education as necessary.
• Follow all agency and regulatory standards for quality care and documentation.

Qualifications:

• Active RN license (in good standing) in the state of practice
• At least one year of recent home health experience; field-based experience highly preferred
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Valid drivers license, reliable transportation, and current auto insurance coverage

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
