Your next nursing opportunity starts here! We are seeking a dedicated Registered Nurse with proven OASIS experience to support patients through in-home visits on a 13-week full-time home health contract. This temporary role is ideal for clinicians who excel in patient-centered care, skilled documentation, and managing a structured schedule while contributing to meaningful patient outcomes.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions tailored to each patient
- Perform comprehensive assessments and complete OASIS documentation accurately and promptly
- Collaborate closely with physicians and interdisciplinary care teams to deliver individualized care plans
- Administer wound care, medication management, patient and family education, and monitor disease progression
- Maintain open communication regarding patient status and care plan updates with the clinical team
Qualifications:
- Active and unrestricted RN license
- At least 1 year of recent experience as a home health RN
- Demonstrated proficiency with OASIS documentation
- Experience with HomeCare HomeBase strongly preferred
- Reliable transportation and valid drivers license required
- Ability to meet the demands of a full-time visit schedule
