Job Description

An opportunity exists for a skilled and compassionate Registered Nurse specializing in Home Health to deliver exceptional patient care in a field-based setting. This role involves providing comprehensive nursing services to patients within their homes, ensuring individualized care plans are followed, and collaborating with interdisciplinary teams to support patient well-being.

Key Responsibilities:

  • Perform detailed OASIS assessments and develop personalized care plans.
  • Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify changes in conditions promptly, and communicate with physicians and care teams.
  • Educate patients and families on health conditions, medication use, and preventive care strategies.
  • Maintain precise documentation using Home Care Home Base (HCHB) software.
  • Work collaboratively with therapists, social workers, and home health aides to ensure cohesive care delivery.
  • Participate in case conferences, staff meetings, and continuing education activities.
  • Comply with all regulatory and agency standards in nursing practice.

Qualifications and Desired Experience:

  • Valid RN license in good standing within the state of practice.
  • At least one year of recent experience in home health nursing, preferably with field assignments.
  • Proficiency with Home Care Home Base (HCHB) and experience with OASIS assessment tool.
  • Strong clinical judgment, critical thinking, and organizational skills.
  • Reliable personal transportation with valid drivers license and auto insurance.
  • Ability to work independently and adapt to a flexible, patient-focused schedule.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Experience managing caseloads of 2532 points weekly.
  • Familiarity with Medicare guidelines and home health compliance.

Schedule and Location:

  • Full-time, 13-week contract assignment.
  • Monday through Friday, 8:00 AM to 5:00 PM.
  • Caseload of 2532 points weekly with approximately 35 patient visits per day.
  • Daily travel within the assigned territory with mileage reimbursement.
