Step into an impactful nursing role by providing essential care to patients within their homes. This full-time, 13-week contract position focuses on home health and is best suited for experienced RNs eager to deliver skilled nursing across a diverse caseload.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Youll make approximately 35 patient visits daily, reaching a caseload of 30 points per week. The role is field-based, requiring daily travel within an assigned territory (with mileage reimbursement provided).

Qualifications:

• Active RN license (in good standing) in the state of practice
• At least 1 year of recent home health nursing experience (field settings strongly preferred)
• Proficient in Home Care Home Base (HCHB) electronic documentation
• Familiar with OASIS assessment tools
• Reliable transportation, valid drivers license, and auto insurance

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop tailored care plans
• Deliver skilled nursing interventions: wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, promptly identifying and communicating any changes with care teams or physicians
• Provide education to patients and their families regarding disease processes, medications, and preventive health
• Maintain detailed, accurate documentation in HCHB
• Collaborate with therapy and social work professionals to ensure coordinated, compassionate care
• Participate in team meetings, case conferences, and ongoing education
• Uphold agency and regulatory standards at all times

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
