Your next nursing opportunity awaits with a rewarding role for an experienced Registered Nurse specializing in home health. Utilize your clinical expertise, documentation accuracy, and organizational skills in a full-time, 13-week contract serving patients in their own homes. Schedule & Commitment:
- 13-week contract, with strong potential for extension
- Full-time, Monday through Friday
- Daily structured patient visit expectations
- On-call duties may be required
Qualifications & Experience:
- Active RN license
- At least 1 year of recent home health experience as an RN
- OASIS documentation expertise required
- Proficiency with HomeCare HomeBase preferred
- Ability to maintain a reliable schedule of daily patient visits
- Valid drivers license and dependable transportation
Primary Responsibilities:
- Deliver professional in-home nursing care and skilled interventions
- Perform comprehensive patient assessments and accurately complete OASIS documentation
- Develop and implement individualized care plans in coordination with physicians and care teams
- Provide wound care, medication management, disease monitoring, and patient/family education
- Communicate effectively with the clinical team regarding patient progress and changes in condition
