Job Description

A rewarding opportunity is available for a registered nurse experienced in home health nursing to provide skilled in-home care on a full-time contract basis. This role focuses on delivering comprehensive patient assessments, accurate documentation, and individualized care plans to improve patient outcomes.

Key Responsibilities:

  • Administer skilled nursing care and perform interventions in patients homes
  • Conduct thorough patient assessments and complete OASIS documentation with precision
  • Collaborate closely with physicians and multidisciplinary care teams to tailor treatment plans
  • Perform wound care, medication management, patient education, and disease monitoring
  • Communicate effectively with clinical teams about patient status and changes in condition

Qualifications and Desired Experience:

  • Active RN license is required
  • Minimum of 1 year recent experience in home health nursing
  • Demonstrated proficiency with OASIS documentation
  • Experience with HomeCare HomeBase is highly preferred
  • Ability to manage a structured, full-time schedule with daily patient visits
  • Reliable transportation and a valid drivers license are necessary
