A skilled Home Health Registered Nurse (RN) is needed to deliver exceptional patient-centered care in a community setting. This full-time, 13-week contract position involves providing skilled nursing services within patient homes, ensuring each individual receives compassionate, thorough, and quality care. The role requires strong clinical judgment, effective communication, and the ability to work independently while coordinating with a multidisciplinary healthcare team.

Schedule and Workload:

• Monday through Friday, 8:00 AM to 5:00 PM
• Manage 2532 points per week, typically 35 patient visits per day
• Field-based with daily travel to patient homes (mileage reimbursed)

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Administer skilled nursing care: wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, identify changes, and communicate promptly with healthcare teams and physicians
• Educate patients and families on treatment plans, medications, and preventive care
• Maintain accurate documentation using Home Care Home Base (HCHB)
• Collaborate with therapists, social workers, and aides to ensure coordinated care
• Comply with all regulatory and agency standards

Qualifications and Experience:

• Valid and active RN license in good standing for the state of practice
• Minimum 1 year of recent home health nursing experience (field experience preferred)
• Proficient with Home Care Home Base (HCHB) and OASIS assessment tool
• Strong clinical assessment and critical thinking capabilities
• Reliable transportation with a valid drivers license and auto insurance
• Excellent organizational, communication, and time management skills
• BSN degree preferred
• Prior experience managing a caseload of 2532 points
• Knowledge of Medicare and home health compliance standards is a plus
