This full-time, 13-week contract offers the opportunity to make a meaningful impact in patients livesproviding skilled nursing in their own environment while leveraging your clinical expertise and assessment skills.

Schedule & Work Environment:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based: daily travel to patient homes within the assigned territory
• Caseload: 30 points per week (approx. 35 patient visits/day)
• Electronic Documentation: Home Care Home Base (HCHB)
• Assessment: OASIS

Key Responsibilities:

• Conduct detailed OASIS assessments and develop individualized care plans
• Deliver expert nursing care including wound management, medication administration, IV therapy, and chronic disease support
• Monitor and evaluate patient progress, communicating significant changes to the care team and physicians
• Educate patients and caregivers on health conditions, treatment regimens, medications, and preventative care
• Maintain thorough, timely documentation using Home Care Home Base
• Collaborate closely with multi-disciplinary team members for coordinated care
• Engage in case conferences, staff meetings, and ongoing professional development
• Adhere to regulatory standards and best practices for home health care

Qualifications:

• Active RN license in good standing for the state
• At least 1 year of recent home health experience (field preferred)
• Proficiency in Home Care Home Base (HCHB) and OASIS documentation
• Reliable transportation, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
