Schedule:
- Full-time, Monday through Friday
- On-call availability probable
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care, including skilled interventions and patient education
- Perform comprehensive patient assessments and complete OASIS documentation accurately and timely
- Create and manage individualized care plans in collaboration with physicians and a multidisciplinary team
- Administer wound care, handle medication management, and monitor chronic conditions
- Communicate essential updates, changes in patient condition, and coordinate with the clinical team for optimal outcomes
Qualifications:
- Active RN licensure
- Minimum one year of recent home health RN experience
- Essential OASIS documentation experience
- Proficiency with HomeCare HomeBase is strongly preferred
- Reliable transportation and valid drivers license
- Able to maintain a set, full-time schedule and meet daily visit requirements
