A skilled Registered Nurse (RN) with OASIS proficiency is needed for a 13-week full-time home health contract. Use your clinical expertise, accuracy in documentation, and efficient care management to make a difference with patients in their own homes.

Schedule:

• 13-week contract, with the possibility of extension
• Full-time, Monday through Friday
• On-call coverage may be required

Key Responsibilities:

• Deliver comprehensive in-home nursing care and skilled interventions
• Accurately complete patient assessments and OASIS documentation within required timelines
• Collaborate closely with physicians and interdisciplinary care teams to implement individualized care plans
• Execute wound care, medication management, patient teaching, and ongoing disease monitoring
• Communicate significant changes and updates to the clinical team promptly

Required Qualifications:

• Active RN license (unrestricted)
• At least one year of recent RN home health experience
• OASIS documentation experience
• Familiarity with HomeCare HomeBase is strongly preferred
• Consistent reliability and ability to manage daily, structured patient visit schedules
• Valid drivers license and reliable transportation
