This full-time, 13-week travel contract is an excellent opportunity for registered nurses seeking career growth. Schedule & Assignment Details:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 30 points per week (typically 35 patient visits per day)
- Field-baseddaily travel to patient homes within assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized plans of care
- Provide skilled nursing services: wound care, IV therapy, medication administration, chronic disease management
- Monitor patient progress; communicate changes to physicians and the care team
- Educate patients and families about disease processes, medications, and care strategies
- Maintain accurate documentation in Home Care Home Base (HCHB)
- Collaborate with PT, OT, MSW, and aides to ensure coordinated delivery of care
- Participate in case conferences and educational opportunities
- Ensure all care meets regulatory and agency standards
Qualifications:
- Hold an active RN license for Pennsylvania (in good standing)
- Minimum 1 year recent Home Health (field) experience required
- Proficiency with HCHB and OASIS mandatory
- Reliable transportation, valid drivers license, and valid auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
