Advance your nursing career by supporting patients in their homes, delivering high-quality care, and ensuring continuity of treatment. This full-time, 13-week contract position is designed for a skilled Registered Nurse with a passion for home health and direct patient interaction.

Schedule & Work Environment:

• Monday to Friday, 8:00 AM 5:00 PM
• Field-based, visiting patients in their homes daily
• Expected caseload: 30 points per week (typically 35 patient visits each day)

What Youll Bring:

• Active, unrestricted RN license in the state of practice
• At least 1 year of recent home health nursing experience
• Demonstrated proficiency with Home Care Home Base (HCHB) for documentation
• OASIS assessment expertise
• Reliable transportation, a valid drivers license, and current auto insurance

Key Responsibilities:

• Perform thorough OASIS assessments and craft personalized care plans
• Deliver skilled nursing, including wound care, medication administration, IV therapy, and chronic disease management
• Ensure regular evaluation of patient progress and promptly communicate changes to the healthcare team and physicians
• Provide patient and family education regarding diagnoses, medication, and preventive care
• Maintain accurate and timely documentation in HCHB
• Collaborate with multidisciplinary team membersphysical and occupational therapists, social workers, and aidesfor coordinated patient care
• Participate in case conferences, staff meetings, and professional development activities
• Consistently follow best practices and regulatory standards in patient care delivery

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
