Deliver exceptional care where it matters mostright at home. This full-time, 13-week contract opportunity is ideal for experienced RNs dedicated to making a difference in patients lives through home health. Work Monday through Friday, 8:00 AM 5:00 PM, managing a caseload of approximately 30 points weekly (averaging 35 patient visits per day) in a field-based setting.
Key Qualifications:
- Active RN license in good standing for the state of practice
- Minimum of one year recent Home Health experience, with preference for field-based roles
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- OASIS assessment experience required
- Reliable transportation, valid drivers license, and current auto insurance
Schedule:
- Monday Friday
- 8:00 AM 5:00 PM
Primary Duties:
- Complete detailed OASIS assessments and design individualized care plans
- Deliver skilled nursing care, such as wound care, medication administration, IV therapy, and chronic disease management in patient homes
- Monitor patient progress, report changes in condition, and collaborate with the care team and physicians
- Educate patients and families on diagnoses, medications, and preventive care strategies
- Accurately document visits and interventions using HCHB
- Collaborate with therapists, social workers, and aides to ensure coordinated care
- Attend case conferences, staff meetings, and participate in ongoing education
- Adhere to all regulatory and documentation requirements

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
