Your next nursing opportunity awaits as we seek a skilled Registered Nurse with OASIS experience for a 13-week full-time home health contract. This pivotal role is ideal for those confident in their clinical expertise, accuracy in documentation, and commitment to delivering outstanding patient care in a home setting.
Schedule:
- 13-week contract, with the potential for extension
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily structured patient visit schedule
Key Responsibilities:
- Deliver comprehensive in-home nursing care and hands-on clinical interventions
- Complete thorough patient assessments, ensuring timely and accurate OASIS documentation
- Develop and implement individualized care plans in collaboration with physicians and care teams
- Provide wound care, medication administration, patient and family education, and disease process monitoring
- Maintain clear and proactive communication with the clinical team about patient status changes
Qualifications:
- Current, active RN license
- Minimum of 1 year of recent home health RN experience
- Demonstrated proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase strongly preferred
- Valid drivers license and reliable transportation for home visits
- Excellent organizational and time management skills to maintain a structured daily schedule
