Your next nursing opportunity begins with a rewarding home health assignment focused on delivering compassionate, patient-centered care. Currently seeking an experienced Registered Nurse with strong OASIS documentation skills for a 13-week full-time contract serving the local community.
Schedule:
- Full-time, Monday through Friday
- On-call coverage may be required
- Daily visit expectations, structured caseload
Key Responsibilities:
- Deliver skilled nursing care and clinical interventions in patient homes
- Complete thorough patient assessments and OASIS documentation accurately and promptly
- Work as part of an interdisciplinary care team, developing and executing individualized care plans
- Provide wound care, medication management, illness monitoring, and patient/family education
- Clearly communicate any patient status updates and changes in condition to the broader care team
Who You Are:
- Active and unrestricted RN license
- At least 1 year of recent RN home health experience
- Proficient with OASIS documentation (required)
- Previous HomeCare HomeBase EMR experience is strongly preferred
- Able to organize and manage a structured full-time schedule with daily patient visits
- Dependable transportation and valid drivers license
