We are seeking a dedicated Registered Nurse with proven OASIS experience to deliver exceptional patient care during a 13-week full-time contract. Take pride in providing clinical excellence and supporting individuals in the comfort of their homes.
Schedule/Hours:
- Full-time, Monday through Friday
- On-call rotation may be required
Key Responsibilities:
- Deliver in-home nursing care, skilled interventions, and personalized education
- Complete patient assessments and thorough OASIS documentation accurately and on schedule
- Collaborate with physicians and multidisciplinary teams to implement tailored care plans
- Manage wound care, medication administration, disease monitoring, and health education
- Communicate pertinent changes in patient condition to the clinical team promptly
- Maintain organized, timely, and compliant documentation for each visit
Qualifications:
- Current, active RN license
- At least 1 year of recent home health RN experience
- Demonstrated expertise with OASIS documentation
- Experience with HomeCare HomeBase strongly preferred
- Ability to independently manage a structured daily caseload
- Reliable transportation and valid drivers license required
