We are seeking a dedicated Registered Nurse (RN) with a strong background in home health and OASIS documentation for a 13-week full-time contract assignment. This role offers the chance to make a meaningful difference by providing critical care to patients in their own homes, while working alongside a professional, supportive team.
Schedule:
- 13-week contract, with potential for extension
- Full-time, Monday through Friday
- On-call responsibilities may apply
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions
- Complete comprehensive patient assessments and accurately document using OASIS
- Collaborate with physicians and multidisciplinary teams to develop personalized care plans
- Provide wound care, medication administration, patient/caregiver education, and ongoing disease monitoring
- Communicate patient status updates and changes to the clinical team promptly
Preferred Qualifications:
- Active RN license in good standing
- Minimum one year of recent experience as a home health RN
- OASIS documentation expertise required
- Proficiency with HomeCare HomeBase (strongly preferred)
- Strong organizational skills to effectively manage daily patient visits and a structured schedule
- Reliable transportation and valid drivers license
