Job Description

This position is tailored for clinicians committed to delivering outstanding care in patients homes, with a special emphasis on accurate OASIS documentation and effective coordination within a multidisciplinary team.

Schedule:

  • Full-time, Monday through Friday
  • On-call responsibilities may be required
  • Structured daily visit expectations over a 13-week contract (with the potential for extension)

Core Responsibilities:

  • Deliver skilled nursing interventions and care within home environments
  • Perform thorough patient assessments, completing OASIS documentation with accuracy and timeliness
  • Oversee wound care, medication management, disease monitoring, and patient education
  • Act as an essential liaison, communicating patient updates and changes in condition to the clinical team
  • Collaborate with physicians, therapists, and other care professionals to customize and deliver thorough care plans

Candidate Profile:

  • Current unrestricted RN license
  • At least one year of recent RN experience in home health care
  • Demonstrated experience with OASIS documentation (required)
  • Proficient in HomeCare HomeBase software (strongly preferred)
  • Strong organizational skills to manage a full-time caseload independently
  • Possess reliable transportation and a valid drivers license
