This position is tailored for clinicians committed to delivering outstanding care in patients homes, with a special emphasis on accurate OASIS documentation and effective coordination within a multidisciplinary team.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily visit expectations over a 13-week contract (with the potential for extension)
Core Responsibilities:
- Deliver skilled nursing interventions and care within home environments
- Perform thorough patient assessments, completing OASIS documentation with accuracy and timeliness
- Oversee wound care, medication management, disease monitoring, and patient education
- Act as an essential liaison, communicating patient updates and changes in condition to the clinical team
- Collaborate with physicians, therapists, and other care professionals to customize and deliver thorough care plans
Candidate Profile:
- Current unrestricted RN license
- At least one year of recent RN experience in home health care
- Demonstrated experience with OASIS documentation (required)
- Proficient in HomeCare HomeBase software (strongly preferred)
- Strong organizational skills to manage a full-time caseload independently
- Possess reliable transportation and a valid drivers license
